The Delirium Diagnostic Tool-Provisional (DDT-Pro) accurately identifies delirium in acquired brain injury (ABI) patients. This new tool shows high validity and correlation with existing measures, supporting its use in clinical settings.
Area of Science:
- Neuroscience
- Clinical Diagnostics
- Psychiatry
Background:
- Delirium is a common and serious complication following acquired brain injury (ABI).
- Accurate and timely diagnosis of delirium is crucial for effective management and improved patient outcomes.
- Existing diagnostic tools may have limitations in specific patient populations like those with ABI.
Purpose of the Study:
- To evaluate the accuracy and validity of the newly developed Delirium Diagnostic Tool-Provisional (DDT-Pro).
- To assess the DDT-Pro's performance in a population of patients with acquired brain injury (ABI).
Main Methods:
- A cross-sectional study design was employed.
- Thirty-six patients with ABI (traumatic brain injury and intraparenchymal hemorrhage) were assessed upon admission to inpatient rehabilitation.
- The DDT-Pro was administered alongside the Delirium Rating Scale-Revised 98 (DRS-R98) and DSM-IV-TR criteria.
Main Results:
- The DDT-Pro demonstrated high accuracy, with an Area Under the Curve (AUC) of 0.994 via receiver operating characteristic (ROC) analysis.
- The tool correctly classified 97% of observations against the DSM-IV-TR gold standard.
- Strong correlations were found between the DDT-Pro and the DRS-R98, both for raw scores (r = -0.913) and delirium duration (r = 0.975).
Conclusions:
- The Delirium Diagnostic Tool-Provisional (DDT-Pro) is a validated measure for detecting delirium in patients with acquired brain injury.
- Further validation studies in broader ABI and other medical populations are recommended to support its widespread clinical application.
