Certain NOD2 gene variations may influence Common Variable Immunodeficiency Disorders (CVID) complications like enteropathy and autoimmune disorders. These NOD2 polymorphisms do not increase CVID susceptibility itself.
Area of Science:
- Immunology
- Genetics
- Internal Medicine
Background:
- Common Variable Immunodeficiency Disorders (CVID) are characterized by hypogammaglobulinaemia, leading to infections.
- CVID patients often develop additional complications such as granulomatous disease and enteropathy, with unclear causes.
- Identifying disease modifier genes is crucial for predicting and managing CVID complications.
Purpose of the Study:
- To investigate if NOD2 gene polymorphisms act as disease modifiers for enteropathic or granulomatous phenotypes in CVID.
- To analyze the association between specific NOD2 mutations and clinical manifestations in a large CVID cohort.
Main Methods:
- Genotyping of 285 CVID patients from UK and European centers using sequence-specific primers.
- Analysis of NOD2 polymorphisms (gly908arg, leu1007finsc, arg702trp) and their correlation with CVID phenotypes.
- Comparison of allele frequencies with UK immunocompetent controls.
Main Results:
- Arg702trp polymorphisms were less frequent in CVID patients with splenomegaly (P=0.038).
- Gly908arg polymorphisms were more prevalent in UK CVID patients with autoimmune disorders or enteropathy (P=0.049).
- NOD2 polymorphisms were not significantly associated with granulomatous phenotypes in CVID patients.
Conclusions:
- Specific NOD2 polymorphisms, particularly gly908arg, may be associated with enteropathy and autoimmune disorders in CVID patients.
- NOD2 polymorphisms do not appear to confer susceptibility to CVID itself.
- Further research into NOD2 as a modifier gene could improve clinical management of CVID complications.
