  • A 42-year-old woman presented with seizures and unconsciousness due to severe hypoglycemia (25 mg/dl) three weeks postpartum.
  • Diagnostic workup revealed hyperinsulinemic hypoglycemia consistent with Whipple's triad, despite negative imaging (CT, MRI, DOTATOC-PET).
  • Endoscopic ultrasound identified a 13 mm pancreatic tail mass.

Findings:

  • Laparoscopic enucleation successfully removed the insulinoma.
  • Postoperative blood glucose levels normalized, and the patient recovered fully.
  • Histological examination confirmed a pancreatic insulinoma.

Implications:

  • This case underscores the importance of considering insulinoma in the differential diagnosis of pregnancy-associated hypoglycemia.
