Reporter Genes

Reporter genes are a type of protein-coding gene that are often tagged to a gene of interest. Once inside a target cell, reporter genes usually produce visually identifiable characteristics like fluorescence and luminescence when expressed along with the gene of interest. Thus, reporter genes “report” the presence or absence of genes of interest in an organism, determine the gene expression pattern, or track the physical location of a DNA segment or protein in the cell.

Background:

Hypoglycemia requires careful diagnosis to distinguish between true and artifactual causes.
Elevated leukocyte counts can interfere with blood glucose measurements.

Observation:

A 39-year-old female patient with lung cancer presented with a venous blood glucose of <0.1 mmol/L and capillary glucose of 4.7 mmol/L.
The patient was asymptomatic despite the critically low venous glucose reading.
A significantly elevated leukocyte count (86.4 G/L, 92% neutrophils) was noted, potentially linked to a paraneoplastic syndrome.

Findings:

In vitro glucose consumption by leukocytes between blood sampling and laboratory analysis caused the discrepancy.
This artifactual hypoglycemia was identified through communication between the biologist and clinician.

Implications:

This case underscores the need to investigate artifactual hypoglycemia, particularly in patients with high leukocyte counts.
Accurate diagnosis of hypoglycemia is crucial for appropriate patient management and treatment.
Interdisciplinary communication is vital for resolving complex diagnostic challenges in clinical practice.
