Background:

  • Traditional surgical redo procedures may be prohibitive due to comorbidities like scoliosis and cardiac cachexia.
  • This case highlights the limitations of conventional treatment in complex anatomical scenarios.

Observation:

  • A frail septogenarian with severe mitral regurgitation and high surgical risk (Logistic EuroSCORE 36.4%, STS score 15.5%) presented with NYHA class IV symptoms.
  • The patient experienced hemolysis requiring frequent blood transfusions, complicating her clinical picture.
  • Open surgical repair was deemed excessively risky, leading to consideration of alternative interventions.

Findings:

  • Amplatzer transcatheter paravalvular leak repair was successfully performed using multiple devices, overcoming anatomical challenges.
  • Post-procedure, the patient showed marked symptom improvement from NYHA IV to NYHA II.
  • Hemolysis persisted but transfusion frequency decreased; LV dimensions improved, and trace mitral regurgitation was sustained at 6 months.

Implications:

  • Transcatheter paravalvular leak repair is feasible and effective for complex cases in extremely high-risk patients.
  • Multiple devices and judicious staging may be necessary to achieve optimal outcomes.
  • This minimally invasive approach provides a valuable alternative when surgical options are limited, improving patient quality of life.
