Background:

  • Retrobulbar hemorrhage is a known complication after intraocular surgery.
  • Lateral canthal procedures, such as canthotomy and cantholysis, are sometimes necessary in oculoplastic cases.

Observation:

  • A 76-year-old male presented with retrobulbar hemorrhage post-intraocular surgery.
  • A canthotomy and inferior cantholysis were performed, leading to a secondary orbital hematoma 14 days later in the canthotomy region.

Findings:

  • The secondary hematoma resolved with conservative management, including warm compresses and observation.
  • Delayed closure of the initial canthotomy wound might have prevented the secondary hematoma and potential visual complications.

Implications:

  • Ophthalmologists must recognize the risk of bleeding after lateral canthal procedures, especially in patients with prior hemorrhagic issues.
  • Awareness of delayed bleeding is crucial for managing patients undergoing eyelid and orbital surgery to prevent vision loss.
