    Background:

    • A 72-year-old male smoker presented with a large right middle lobe lung tumor (12x8x8 cm).
    • Metastatic spread was observed in regional lymph nodes, abdominal wall soft tissue, gastric mucosa, and the left adrenal gland.
    • Electron microscopy revealed tumor cells with diverse differentiation, including squamous, glandular, smooth muscle, and fibroblastic features.

    Findings:

    • The tumor demonstrated a biphasic morphology, with components of undifferentiated carcinoma, adenocarcinoma, and leiomyosarcoma.
    • The epithelial component showed features of poorly differentiated carcinoma and adenocarcinoma.
    • The mesenchymal component was identified as leiomyosarcoma.

    • This case highlights the complex nature of lung carcinoma with divergent differentiation.
    • The extensive metastatic pattern underscores the aggressive potential of such tumors.
