Area of Science:

  • Orthopaedics
  • Rheumatology
  • Podiatry

Background:

  • Rheumatoid arthritis frequently causes forefoot deformities and pain, impacting patient function.
  • Despite advances in medical management, surgical intervention is often necessary for severe deformities.
  • Common deformities include hallux valgus and metatarsophalangeal (MTP) joint subluxation/dislocation.

Purpose of the Study:

  • To review the pathophysiology of rheumatoid forefoot deformity.
  • To emphasize recent advancements in surgical management strategies.
  • To highlight the importance of a multidisciplinary approach.

Main Methods:

  • Review of current literature on rheumatoid forefoot deformity.
  • Analysis of surgical techniques and outcomes.
  • Emphasis on the gold standard procedure for comparison.

Main Results:

  • Forefoot deformity in rheumatoid arthritis involves hallux valgus and MTP joint issues.
  • Surgical goals include correction, decompression, and achieving a plantigrade foot.
  • First MTP joint fusion with lesser MTP joint excision arthroplasty is the established standard.

Conclusions:

  • Successful management requires a multidisciplinary, integrated approach.
  • Surgical intervention is crucial for correcting deformities and improving function.
  • Established surgical techniques provide a benchmark for evaluating newer procedures.
