Summary

A new image recognition system accurately confirms endotracheal tube placement using a mannequin model. This automated system shows 100% success in distinguishing correct tracheal intubation from esophageal intubation.

Background:

  • Endotracheal intubation is a critical procedure for airway management.
  • Accurate confirmation of endotracheal tube (ETT) placement is essential to prevent complications.
  • Current confirmation methods can be subjective or time-consuming.

Purpose of the Study:

  • To introduce and evaluate a novel system for accurate endotracheal intubation positioning confirmation.
  • To assess the performance of an image classification algorithm for differentiating tracheal from esophageal intubation.

Main Methods:

  • A system utilizing a miniature complementary metal-oxide-semiconductor (CMOS) sensor on a stylet was developed.
  • Video signals were acquired by a digital signal processor (DSP) and processed by an image classification algorithm.
  • A leave-one-out cross-validation method was used to test the algorithm on 150 images from mannequin intubations.

Main Results:

  • The system correctly identified 80 out of 80 endotracheal intubations.
  • The system correctly identified 70 out of 70 esophageal intubations.
  • The image recognition system demonstrated a 100% success rate in discriminating correct ETT positioning on a mannequin.

  • A fully automatic image recognition system successfully confirmed endotracheal tube positioning.
  • The developed system shows promise for real-time, accurate ETT placement verification.
  • Further research involving live tissue models and human trials is warranted.
