Summary

Acute disseminated encephalomyelitis (ADEM) can rapidly cause severe brain swelling. Emergency surgery, like hemicraniectomy, may be life-saving for ADEM patients with hyperacute cerebral edema.

Area of Science:

  • Neurology
  • Neuroscience
  • Demyelinating Diseases

Background:

  • Acute disseminated encephalomyelitis (ADEM) is a rare neurological disorder characterized by acute demyelination.
  • ADEM typically presents subacutely but can manifest with rapid intracerebral mass effect due to cerebral edema.

Observation:

  • A case report details a patient with a hyperacute ADEM presentation.
  • The patient experienced rapid loss of consciousness and focal neurological deficits.

Findings:

  • Emergency hemicraniectomy and steroid treatment led to swift neurological improvement.
  • The patient exhibited minimal long-term neurological deficits after three months.

Implications:

  • Surgical decompression, such as hemicraniectomy, is crucial for life-saving intervention in hyperacute cerebral edema secondary to ADEM.
