Summary

The homozygous deletion genotype of the NFKB1 gene promoter significantly increases the risk of post-tonsillectomy bleeding. This genetic factor nearly quadruples bleeding risk, independent of other common risk factors.

Purpose of the Study:

  • To determine if the NFKB1 gene promoter insertion/deletion polymorphism (-94ins/delATTG) is associated with bleeding risk after tonsillectomy.
  • To identify genetic markers for predicting post-tonsillectomy hemorrhage.

Main Methods:

  • Retrospective study involving 148 patients undergoing tonsillectomy.
  • Genotyping of the NFKB1 promoter -94ins/delATTG polymorphism from tonsillar tissue or blood.
  • Statistical analysis comparing genotype frequencies between patients with and without hemorrhage.

Main Results:

  • The homozygous deletion genotype (DD) was significantly more frequent in patients with post-tonsillectomy hemorrhage (n=56) compared to those without (n=92).
  • Carriers of the DD genotype had an odds ratio of 3.78 for bleeding (P = .023).
  • No significant differences were found in demographic factors, surgical details, medications, or coagulation studies between groups.

Conclusions:

  • The homozygous deletion allele of the NFKB1 promoter polymorphism is an independent risk factor for post-tonsillectomy hemorrhage.
  • Individuals with this genotype have a nearly fourfold increased risk of bleeding.
  • Genetic screening may aid in identifying patients at higher risk for post-operative bleeding.
