Endoscopic Procedures V: ERCP01:26

Endoscopic Procedures V: ERCP

Endoscopic Retrograde Cholangiopancreatography (ERCP) is a diagnostic procedure that combines endoscopy and fluoroscopy to diagnose and treat conditions related to the bile ducts, pancreatic ducts, and gallbladder. This procedure is beneficial for identifying and addressing blockages, gallstones, strictures, and tumors within the biliary or pancreatic systems. ERCP is both diagnostic and therapeutic, offering the ability to visualize and treat identified problems in one session.

Urinary Tract Calculi VI: Surgical Management

Procedures for Kidney StonesMedical intervention is necessary when kidney stones or renal calculi are too large to pass spontaneously (typically greater than 5 millimeters) when stones are accompanied by symptomatic infection (such as fever or pyelonephritis), when they impair kidney function, or when they cause persistent symptoms like severe pain, nausea, or urinary retention. Additionally, patients with only one kidney or those who cannot be treated with medical management also require...

Urologic Endoscopic Procedure: Cystoscopic Examination

Meaning of Cystoscopic Examination:Cystoscopy is an essential diagnostic tool in urology that is used to assess the structure and function of the genitourinary system. It provides a direct view of the urethra, bladder, and, in some cases, the ureteral openings. This procedure helps detect structural abnormalities, infections, cancers, and blockages in the urinary tract. There are two types of cystoscopy:Flexible cystoscopy is commonly performed in outpatient settings due to its less invasive...

Endoscopic Procedures IV: Sigmoidoscopy and Laproscopy

Sigmoidoscopy and laparoscopy are distinct medical procedures that enable physicians to internally inspect different parts of the GI tract. Although they serve different purposes, each is essential for diagnosing and, in some cases, treating various medical conditions.
Sigmoidoscopy
Sigmoidoscopy is a diagnostic procedure that uses a flexible sigmoidoscope equipped with a light source and camera to examine the rectum and sigmoid colon. The procedure involves inserting the tube through the anus...

Summary

For choledocholithiasis, surgery is an effective treatment. While endoscopic retrograde cholangiopancreatography (ERCP) and endotherapy were attempted, surgery was ultimately required for most patients, demonstrating its efficacy and safety.

Area of Science:

  • Gastroenterology
  • Surgical Gastroenterology

Background:

  • Optimal treatment for choledocholithiasis remains debated.
  • Treatment decisions are influenced by available expertise and scientific evidence.

Purpose of the Study:

  • To evaluate the efficacy of endotherapy versus surgery for choledocholithiasis.
  • To compare treatment outcomes in patients with common bile duct stones.

Main Methods:

  • Retrospective analysis of 349 patients with gallstone disease from February 2005 to January 2010.
  • Focused analysis on 56 patients with choledocholithiasis treated with preoperative ERCP and endotherapy followed by cholecystectomy.

Main Results:

  • Endotherapy was successful in 15 out of 56 patients (26.8%).
  • Surgery was required for the remaining 41 patients (73.2%).
  • Surgical intervention demonstrated acceptable morbidity and no mortality.

Conclusions:

  • Surgery is an efficacious and safe option for choledocholithiasis, even in complex cases.
  • While endotherapy has a role, surgical management remains crucial for successful treatment of common bile duct stones.
