Purpose:

To establish a formal 'experience feedback committee' (comité de retour d’expérience [Crex]) within radiotherapy departments.
To integrate the human factor as a key element in systemic deviation analysis.
To create a collective framework for analyzing incidents and implementing corrective actions.

Summary:

The experience feedback committee (Crex) systematically reviews monthly incidents and precursors.
It utilizes the Orion method for in-depth systemic analysis of selected events.
The committee ensures the selection and implementation of appropriate corrective actions, approved by the nuclear safety authority (ASN).
