Dental implants can be successfully placed in atrophic maxillae using the osteotome sinus floor elevation (OSFE) technique without grafting. This method simplifies rehabilitation and achieves stable bone formation over five years.

  • All implants survived and demonstrated peri-implant bone gain (mean increase 3.2±1.3mm).
  • Implant protrusion into the sinus significantly reduced from 4.9±1.9mm to 1.5±0.9mm over 5 years.
  • Mean crestal bone loss of 0.8±0.8mm stabilized, with 20 implants showing further bone gain post-1 year.

Conclusions:

  • Implant rehabilitation of atrophic maxillae can be simplified using 10mm implants and the OSFE technique without grafting.
  • Grafting material is unnecessary for achieving at least 3mm of bone gain.
  • The procedure is predictable and yields favorable long-term outcomes.
