Background:

Behavioral and psychological symptoms of dementia (BPSD) present significant challenges for community-dwelling elderly individuals.
Accurate differentiation between dementia subtypes and BPSD is crucial for effective management.

Observation:

The review examines current data on elderly individuals experiencing BPSD.
It proposes conceptual frameworks for distinguishing core dementia symptoms from BPSD, and agitation from delirium.
Two case studies illustrate the risks of over-treating agitation and the importance of differentiating it from delirium in dementia with Lewy bodies (DLB).

Findings:

Misinterpreting behaviors solely as 'agitation' can lead to inappropriate antipsychotic use.
Clear differentiation between agitation and delirium is vital, especially in conditions like DLB.
Over-focusing treatment on agitation risks overlooking underlying causes and exacerbating patient distress.

Implications:

Physicians must prioritize patient-centered care, basing interventions on the afflicted individual's needs, not solely on caregiver burden or family preference.
Improved diagnostic clarity for BPSD and related conditions can lead to more targeted and effective treatment strategies.
This approach can reduce caregiver burden and improve the quality of life for individuals with dementia.
