Area of Science:

  • Neurology
  • Developmental Biology
  • Genetics

Background:

  • Diastematomyelia is a congenital spinal cord malformation.
  • It can present with diverse and sometimes subtle clinical signs.
  • Familial occurrence suggests a potential genetic component.

Observation:

  • A case presented with unilateral, painless extremity ulceration and lumbar hypertrichosis.
  • Radiography revealed a lumbar bony spur and widened interpediculate space.
  • Imaging confirmed spinal cord and dural duplication.

Findings:

  • Diastematomyelia was diagnosed based on clinical presentation and advanced imaging (myelography, CT, MRI).
  • The condition involves duplication of the spinal cord and potentially the dura mater.
  • Surgical treatment led to the resolution of a perforating foot ulcer.

Implications:

  • Highlights the importance of considering spinal dysraphism in cases of familial extremity ulcers.
  • Suggests a link between spinal cord malformations and cutaneous findings.
  • Demonstrates the efficacy of surgical correction for associated ulcerative complications.
