Summary

Simple reaction time (SRT) testing can now effectively identify older adults at higher risk for falls. A new, clinically feasible protocol with a cutoff score of 394.5 ms accurately distinguishes between single and multiple fallers.

Background:

Simple Reaction Time (SRT) is a valid fall predictor but lacks standardized clinical protocols.
Clinical falls assessment batteries often omit SRT due to the absence of feasible testing methods and cutoff scores.

Purpose of the Study:

To establish a clinically viable protocol for measuring SRT.
To determine if SRT scores can differentiate between individuals who have experienced single versus multiple falls in the past year.

Main Methods:

A cohort of 147 older adults (≥65 years) with a history of falls were recruited.
A clinically feasible lower limb SRT testing protocol was administered.
Statistical analyses, including discriminant analysis and ROC curve analysis, were used to evaluate SRT's predictive capability.

Main Results:

SRT scores significantly discriminated between single and multiple fallers (Wilk's λ = 0.89, p = 0.05).
The area under the ROC curve was 0.78, indicating good diagnostic accuracy.
A cutoff score of 394.5 ms yielded an odds ratio of 7.18 for predicting multiple falls, with a positive predictive value of 84.5%.

Conclusions:

The developed lower limb SRT testing protocol is clinically feasible and effectively discriminates between single and multiple fallers.
This SRT test can aid clinicians in identifying older individuals at increased risk of recurrent falls.
Incorporating SRT into clinical practice may improve fall prevention strategies for the elderly.
