Localized photochemotherapy (PUVA) for chronic palm and sole psoriasis can cause pigmented macules. These lesions show diverse histopathology, indicating a broad spectrum of skin reactions to PUVA treatment.

Area of Science:

  • Dermatology
  • Photomedicine
  • Histopathology

Background:

  • Chronic psoriasiform dermatitis of the palms and soles is a challenging condition.
  • Localized psoralen plus ultraviolet A (PUVA) therapy is a treatment option for recalcitrant cases.

Observation:

  • Four patients with chronic palmoplantar psoriasiform dermatitis developed pigmented macular lesions following localized PUVA therapy.
  • Lesions appeared on treated areas of the palms and soles.

Findings:

  • Histopathologic examination revealed a spectrum of changes in the pigmented macules.
  • Diagnoses included lentigines, atypical melanocytic proliferation, and a junction nevus.
  • This suggests a wide range of clinicopathologic presentations for PUVA-induced pigmented macules.

Related Experiment Videos

Implications:

  • Pigmented macules are a potential side effect of localized palmoplantar PUVA therapy.
  • Clinicians should be aware of the varied histopathologic findings associated with these lesions.
  • Further research may clarify the long-term significance and management of these PUVA-induced changes.
