Modern diffuse large B cell lymphoma treatments, including rituximab with CHOP (cyclophosphamide, doxorubicin, vincristine, prednisone), have significantly improved cure rates. Dose-dense regimens show promise, but further research is needed for consensus.
Area of Science:
- Hematology
- Oncology
- Clinical Medicine
Background:
- Diffuse large B cell lymphoma (DLBCL) management has seen significant progress.
- Modern treatment regimens have increased cure rates from 40% to approximately 75%.
Purpose of the Study:
- To review recent advances in DLBCL treatment.
- To highlight areas of consensus and controversy in DLBCL management.
Main Methods:
- Review of clinical data and treatment regimens for DLBCL.
- Comparison of standard CHOP regimen with rituximab-enhanced regimens (R-CHOP) and dose-dense variations (CHOP-14).
Main Results:
- The R-CHOP regimen significantly improves survival compared to CHOP alone.
- Dose-dense CHOP-14 demonstrates superiority over standard CHOP given every 21 days.
Conclusions:
- Consensus exists on using R-CHOP as front-line treatment for DLBCL.
- Maintenance rituximab is not effective for DLBCL, unlike in follicular lymphomas.
- Head-to-head comparison of R-CHOP-14 and R-CHOP-21 is lacking, preventing consensus on the former's superiority.
