Background:

  • Postural orthostatic tachycardia syndrome (POTS) is characterized by excessive heart rate increase upon standing.
  • The role of physical deconditioning versus intrinsic autonomic dysfunction in POTS remains debated.

Purpose of the Study:

  • To investigate whether the exaggerated heart rate response in POTS is primarily due to deconditioning or an independent autonomic feature.
  • To differentiate the physiological characteristics of POTS patients with and without deconditioning.

Main Methods:

  • Retrospective analysis of 202 adolescents undergoing head-up tilt and maximal exercise testing.
  • Classification of POTS based on heart rate increase during tilt-table test (≥ 30 bpm).
  • Classification of deconditioning based on peak oxygen uptake (< 80% predicted).

Main Results:

  • Two-thirds of all patients, including those with and without POTS, were deconditioned.
  • Peak oxygen uptake was similar between POTS and non-POTS groups.
  • POTS patients exhibited higher resting and exercise heart rates, lower stroke volume during exercise, and slower heart rate recovery compared to non-POTS patients.

Conclusions:

  • While deconditioning is common in patients with POTS symptoms, it does not fully account for the observed heart rate abnormalities.
  • The findings suggest that autonomic dysfunction plays a significant role in the pathophysiology of POTS, independent of physical inactivity.
