Community-acquired methicillin-resistant Staphylococcus aureus (CA-MRSA) infections are rising. This case highlights rare pediatric presentations of toxic shock syndrome, necrotizing fasciitis, and symmetrical peripheral gangrene caused by CA-MRSA.

Background:

  • Pediatric cases of severe CA-MRSA manifestations like toxic shock syndrome (TSS), necrotizing fasciitis (NF), and symmetrical peripheral gangrene (SPG) are uncommon.

Observation:

  • A pediatric patient presented with a history of trauma.
  • The patient subsequently developed severe clinical manifestations including TSS, NF, and SPG.

Findings:

  • Causative agent identified as CA-MRSA.
  • The clinical presentation included a rare triad of severe invasive infections.

Implications:

  • CA-MRSA should be considered in the differential diagnosis for pediatric patients presenting with severe systemic illness and skin/tissue necrosis.
  • Prompt and aggressive surgical debridement and antibiotic therapy are crucial for favorable outcomes in these rare pediatric CA-MRSA infections.
