Summary

A failing root canal treatment in one tooth can cause inflammatory resorption and pulpal necrosis in an adjacent tooth. This case highlights a previously unreported cause of dental inflammatory lesions.

Background:

  • Periapical lesions and perforating resorption defects pose diagnostic challenges in endodontics.
  • The etiology of inflammatory periapical lesions is often attributed to the involved tooth's pulpal status.

Observation:

  • A large periapical lesion with perforating resorption on a cuspid was treated endodontically, with no radiographic improvement after 18 months.
  • Significant healing occurred only after extraction of a neighboring tooth with previous root canal therapy.

Findings:

  • The primary cause of the periapical lesion and resorptive defect was determined to be failing root canal therapy of the adjacent tooth.
  • This case presents a novel finding: inflammatory resorption resulting from the pulpal necrosis of a neighboring tooth.

Implications:

  • Clinicians should consider adjacent teeth with prior endodontic treatment as potential sources of inflammatory periapical pathology.
  • This finding expands the understanding of inflammatory resorption mechanisms in endodontology.
