A large inguinal hernia can cause testicular ischemia, a rare but serious condition. Prompt diagnosis and manual reduction of the hernia can restore blood flow and save the testicle.

  • Repeat scrotal ultrasound confirmed re-established vascular flow to the testicle.

  • This case underscores the importance of considering inguinal hernias in the differential diagnosis of acute scrotal pain.
  • Early recognition and intervention, including hernia reduction, can prevent irreversible testicular damage.
