  • A case of severe congestive left heart failure was observed.
  • The heart failure was directly linked to profound and long-term hypocalcemia.
  • The hypocalcemia was secondary to surgically induced hypoparathyroidism.

Findings:

  • Hypocalcemia should be considered in the differential diagnosis of refractory congestive heart failure.
  • Rapid correction of serum calcium levels can avert unnecessary diagnostic and therapeutic interventions.
  • Heart failure caused by hypocalcemia is completely reversible with calcium and vitamin D supplementation.

  • Clinicians should be vigilant for hypocalcemia in patients with unexplained or treatment-resistant heart failure.
