CD30-positive anaplastic variant diffuse large B cell lymphoma: a rare case presented with cutaneous involvement

Background:

• Diffuse large B cell lymphoma (DLBCL) is an aggressive non-Hodgkin lymphoma.

Observation:

• A 67-year-old male presented with cervical lymphadenopathy and tenderness.
• Biopsy revealed pleomorphic large cells positive for CD30, but negative for B and T cell markers.
• Subsequent skin ulceration showed similar atypical cells in the dermis and epidermis.

Findings:

• Immunohistochemistry confirmed CD30, CD45, and PAX5 positivity in lesional cells.
• Absence of CD3, CD10, CD20, CD15, Bcl-2, EBER ISH, EMA, and ALK supported the diagnosis.
• The patient was diagnosed with CD30-positive DLBCL, anaplastic variant, with nodal and cutaneous involvement.

Implications:

• This case highlights a rare presentation of DLBCL, anaplastic variant, with unusual skin involvement.
