Summary

When one twin dies in utero, the surviving twin faces risks, especially if death occurs later in pregnancy. Early fetal death offers a better prognosis for the surviving twin.

Background:

  • Intrauterine fetal death (IUFD) of one twin in a monochorionic pregnancy can impact the surviving co-twin.

Observation:

  • A study examined 133 monochorionic twin pairs, identifying 33 cases (24.8%) with IUFD of one twin.
  • Among 33 surviving twins, 8 exhibited neurological abnormalities such as porencephaly and cerebral palsy.

Findings:

  • The timing of fetal death significantly influences the prognosis for the surviving twin.
  • Later second-trimester or third-trimester IUFD is associated with a higher risk of adverse outcomes in the survivor.

  • Understanding the risks associated with timing of IUFD can inform clinical management and counseling for affected families.
