Background:

Achieving ideal esthetics in dental implantology requires careful consideration of the extraction socket's soft and hard tissue characteristics.
Predicting and managing esthetic outcomes following tooth extraction and subsequent implant placement remains a clinical challenge.

Purpose of the Study:

To establish clinical criteria for soft and hard tissues of extraction sockets to achieve ideal implant esthetics.
To evaluate the esthetic outcomes of immediate and delayed dental implant placement based on socket morphology.

Main Methods:

Extraction of 25 maxillary anterior teeth, followed by immediate soft and hard tissue assessment of the sockets.
Classification of extraction sockets into adequate, compromised, and deficient types.

Therapeutic approaches guided by a proposed classification, with esthetic outcomes evaluated at prosthesis placement and 1 year post-function.

Main Results:

A treatment decision tree was developed based on socket morphology.
Type I sockets showed adequate esthetics regardless of treatment.
Type II sockets demonstrated better long-term esthetic stability with immediate-delayed implant placement compared to immediate placement.
Type III sockets treated with delayed implantation and grafting achieved 75% adequate esthetic results.

Conclusions:

Treatment planning based on extraction socket tissue morphology significantly enhances the predictability of implant esthetics.
Socket classification provides a valuable tool for guiding therapeutic decisions in implant dentistry.
