Background:

  • Mixed-phenotype acute leukemia (MPAL) is a rare hematologic malignancy.
  • Intraocular involvement in MPAL has not been previously documented.
  • Allogeneic bone marrow transplantation (BMT) is a treatment for acute leukemia.

Observation:

  • A 23-month-old girl with MPAL experienced a relapse after allogeneic BMT.
  • The relapse presented as conjunctivitis, suggesting ocular involvement.
  • A lineage switch in the leukemic cells was identified.

Findings:

  • Clinical, radiologic, and cytologic findings confirmed leukemic intraocular relapse.
  • The relapse demonstrated a switch from the original leukemia phenotype.
  • Fine needle aspiration (FNA) proved effective for diagnosis.
