Panoramic radiographs underestimate bone height in the premolar area. Cone-beam CT is recommended for visualizing anatomical structures before sinus augmentation procedures.

  • Panoramic radiographs have inherent limitations in accurately depicting anatomical structures due to their two-dimensional nature and deformation.
  • Accurate assessment of bone availability and anatomical landmarks is crucial for successful sinus augmentation procedures.

Purpose of the Study:

  • To quantify the underestimation of mesio-distal bone in the premolar region by panoramic radiographs compared to multi-slice/cone-beam CT.
  • To determine the prevalence, dimensions, and position of bony canals in the lateral sinus wall and palatal aspect of the upper canine.

Main Methods:

  • Measurements of bone distance were taken on panoramic radiographs and multi-slice/cone-beam CT (65 patients) at apical, mid-radicular, and crestal regions.
  • The presence and dimensions of intra-osseous canals were assessed on multi-slice CT scans (144 patients) using reformatted images.

Main Results:

  • An intra-osseous canal in the lateral maxillary sinus wall was present in 49.5% of cases (mean diameter 1.4 mm).
  • A bony canal was observed in the palatal aspect of the upper canine in 32.9% of cases (mean diameter 1.23 mm).

Conclusions:

  • Cone-beam CT provides superior visualization of anatomical structures compared to panoramic radiography.
  • Cone-beam CT is recommended for precise anatomical assessment during the planning of sinus augmentation procedures.
