Issues And Trends In Healthcare Delivery System

The issues and trends in healthcare delivery are constantly changing. The COVID-19 pandemic is one recent issue that wreaked havoc on healthcare systems, causing a shortage of healthcare workers, high demand for medicines and supplies, and increased medical expenditure due to a lack of insurance. Other issues include rising healthcare costs and care fragmentation.
Cost Containment
Payment for healthcare services has historically promoted adoption of costly and often unnecessary or inefficient...

Summary

The number of arthroplasty surgeons is projected to be insufficient to meet rising demand for joint replacement surgeries, potentially limiting patient access to care.

Area of Science:

  • Orthopedic Surgery
  • Health Workforce Economics

Background:

  • Arthroplasty (joint replacement) demand is rapidly increasing.
  • There is a declining interest in arthroplasty among surgical residents.
  • Surgeon retirements are further exacerbating potential workforce shortages.

Purpose of the Study:

  • To project the supply of and demand for arthroplasty surgeons in the near future.
  • To determine if the current and projected surgeon workforce will meet the anticipated demand for arthroplasty procedures.
  • To identify potential patient access issues due to surgeon supply shortfalls.

Main Methods:

  • Utilized survey data to estimate the 2016 arthroplasty workforce.
  • Estimated 2016 demand for arthroplasty using the Nationwide Inpatients Sample database.
  • Modeled surgeon supply based on projected new entrants and retirements at different age thresholds (59 and 65 years).

Main Results:

  • Projected a significant procedural shortfall in 2016, ranging from 18.6% to 69.4% depending on retirement age assumptions.
  • Between 2008 and 2016, the workforce was projected to gain 400 specialists and 1584 generalists.
  • Retirements by 2016 were estimated between 1896 (conservative) and 4239 (baseline).

Conclusions:

  • The economic model predicts a critical supply-side crisis in arthroplasty surgery.
  • Patient access to essential joint replacement procedures is under significant threat.
  • Immediate interventions are necessary to increase the supply of arthroplasty surgeons.
