Yersinia pseudotuberculosis can cause severe lower gastrointestinal bleeding in adults. This case highlights the importance of considering this rare bacterial infection in patients with unexplained gastrointestinal hemorrhage.

Background:

  • A 25-year-old male presented with fever, vomiting, body aches, and massive lower gastrointestinal bleeding.
  • Emergency explorative laparotomy revealed localized wall thickening and ulcers in the cecum.
  • Histological examination showed transmural inflammation and non-caseation granulomas in mesenteric lymph nodes.

Findings:

  • Yersinia pseudotuberculosis infection was confirmed by positive serology.
  • The patient underwent a right hemicolectomy and was treated with gentamicin.
  • Histopathology revealed characteristic inflammatory changes consistent with Yersinia infection.

Implications:

  • This case underscores the importance of considering Yersinia pseudotuberculosis in the differential diagnosis of massive lower gastrointestinal bleeding.
  • Early recognition and appropriate antibiotic treatment are crucial for favorable outcomes.
