Endoscopic Procedures IV: Sigmoidoscopy and Laproscopy01:26

Endoscopic Procedures IV: Sigmoidoscopy and Laproscopy

Sigmoidoscopy and laparoscopy are distinct medical procedures that enable physicians to internally inspect different parts of the GI tract. Although they serve different purposes, each is essential for diagnosing and, in some cases, treating various medical conditions.
Sigmoidoscopy
Sigmoidoscopy is a diagnostic procedure that uses a flexible sigmoidoscope equipped with a light source and camera to examine the rectum and sigmoid colon. The procedure involves inserting the tube through the anus...

Cardiomyopathy VII: Pre and Post Operative Nursing Management

Patients with hypertrophic cardiomyopathy (HCM) and left ventricular outflow tract (LVOT) obstruction who remain symptomatic despite optimal medical therapy may undergo a septal myectomy (Morrow procedure). This procedure involves excising a portion of the hypertrophied septum below the aortic valve using a heart-lung machine to improve blood flow through the LVOT. Effective preoperative and postoperative nursing management ensures successful patient outcomes, minimizes complications, and...

Kidney Transplant II: Surgical Procedure

Preoperative ManagementThe primary goals of preoperative management in kidney transplantation are to optimize the patient’s metabolic state and prepare them for surgery through diet adjustments, necessary dialysis, and tailored medical treatment. This phase also involves comprehensive infection screening and patient education about the surgical procedure and postoperative care to improve outcomes and adherence.Medical ManagementA comprehensive evaluation is required for both the living donor...

Peripheral Artery Disease V: Postoperative Nursing Management

During the postoperative period, it is crucial to focus on maintaining circulation, identifying and managing potential complications, and planning for discharge.Nursing AssessmentVital signs monitoring: Regularly monitor vital signs, including blood pressure, heart rate, respiratory rate, and temperature, to detect early signs of complications such as bleeding and infection.Circulation assessment: Monitor pulses, perform Doppler assessments, and check capillary refill, color, temperature, and...

Appendicitis-II: Diagnostic Studies and Management

Diagnosing and managing appendicitis requires a structured and comprehensive approach that spans from initial assessment to postoperative care. Here is an overview of the process:
Diagnosing Appendicitis
It requires a multifaceted approach, starting with a detailed physical examination to pinpoint the location and nature of the pain and identify any associated symptoms. Laboratory tests play a crucial role. A complete Blood Count (CBC) typically reveals leukocytosis (an increased number of...

Peptic Ulcer Disease V: Surgical Management and Nursing Care

Surgical management and nursing care are crucial in treating Peptic Ulcer Disease (PUD). Here is an organized and enhanced overview of the surgical interventions and the associated nursing care for PUD:

Summary

A rare conversion disorder case occurred in a 16-year-old girl post-anesthesia. This severe postoperative presentation highlights the need for awareness of psychological distress manifesting physically after medical procedures.

Area of Science:

  • Psychiatry
  • Neurology
  • Anesthesiology

Background:

  • Conversion disorder, a somatoform illness, involves loss of physical function due to psychological distress.
  • Previously termed hysteria, it is rare, affecting 11-300 per 100,000 people.
  • Post-anesthetic conversion disorder is exceptionally rare, with fewer than 20 documented cases.

Observation:

  • A previously healthy 16-year-old female developed severe symptoms upon emergence from general anesthesia.
  • The patient underwent direct laryngoscopy with vocal fold injection.
  • This case represents a significant deviation from previously reported mild instances in young patients.

Findings:

  • The patient exhibited a severe manifestation of conversion disorder.
  • This occurred subsequent to general anesthesia and a specific surgical procedure.
  • The case is notable for its severity in an adolescent patient.

Implications:

  • This case underscores the potential for severe conversion disorder post-anesthesia, even in adolescents.
  • It highlights the importance of considering psychological factors in unexplained postoperative neurological deficits.
  • Further research into the neurobiological underpinnings and management of post-anesthetic conversion disorder is warranted.
