Summary

Thyroid isthmusectomy may be a sufficient treatment for select patients with small, well-differentiated thyroid cancer (WDTC) confined to the thyroid isthmus. This approach avoids complications associated with more extensive surgeries.

Background:

  • Thyroid cancer treatment guidelines do not specify isthmusectomy.
  • Some thyroid cancers are isolated to the isthmus.
  • Isthmusectomy offers an alternative to total thyroidectomy or extensive dissection.

Purpose of the Study:

  • To analyze outcomes of isthmusectomy for small well-differentiated thyroid cancer (WDTC).

Main Methods:

  • Retrospective analysis of 19 patients with WDTC treated with isthmusectomy.
  • Data collected included demographics, surgical details, pathology, and outcomes.
  • Follow-up ranged from 53 to 276 months.

Main Results:

  • All patients had solitary nodules confined to the isthmus (median size 1 cm).
  • 18 patients had pT1 lesions, one had pT2.
  • No recurrent laryngeal nerve palsy or hypocalcaemia complications.
  • 10-year disease-specific survival was 100%.
  • 10-year local and regional recurrence-free survival was 100%.

Conclusions:

  • Isthmusectomy may be adequate for selected patients with small WDTC of the isthmus.
  • This procedure minimizes risks to the recurrent laryngeal nerve and parathyroid glands.
  • Reduced postoperative complications are a key benefit.
