Diagnosing neurognathostomiasis, a severe complication of gnathostomiasis, can be challenging. New immunoblotting techniques using 21-kD and 24-kD bands show high accuracy for detecting antibodies against Gnathostoma spinigerum.
Area of Science:
- Medical Parasitology
- Neurology
- Infectious Diseases
Background:
- Neurognathostomiasis is a rare and severe manifestation of human gnathostomiasis.
- Current diagnostic methods rely heavily on clinical presentation, with limited serological tests available.
- Accurate diagnosis is crucial for timely and effective treatment of this serious condition.
Purpose of the Study:
- To evaluate the diagnostic utility of specific antibody detection against Gnathostoma spinigerum in patients with neurognathostomiasis.
- To assess the sensitivity and specificity of immunoblotting techniques using 21-kD and 24-kD diagnostic bands.
Main Methods:
- A study was conducted on 12 patients diagnosed with neurognathostomiasis at Srinagarind Hospital, Thailand.
- Patients presented with intracerebral hemorrhage, subarachnoid hemorrhage, or myelitis.
- Antibodies against Gnathostoma spinigerum were tested using an immunoblotting technique with specific diagnostic bands.
Main Results:
- The 21-kD diagnostic band demonstrated 83.3% sensitivity and 100% specificity.
- The 24-kD diagnostic band showed 91.7% sensitivity and 100% specificity.
- Both bands exhibited 100% positive and negative predictive values, indicating excellent diagnostic performance.
Conclusions:
- The 21-kD and 24-kD diagnostic bands are valuable tools for diagnosing neurognathostomiasis.
- These immunoblotting bands possess strong diagnostic properties, aiding in the identification of this rare neurological parasitic infection.
- The findings support the use of these serological markers to improve diagnostic accuracy in neurognathostomiasis cases.

There are many different types of ELISAs, but they all involve an antibody molecule whose constant region binds an enzyme, leaving the variable region free to bind its specific antigen. Enzyme-substrate reaction allows the antigen to be visualized or quantified.
