It is not a specific disease but a syndrome, with Alzheimer's disease being the most common cause, accounting for approximately 60-80% of cases. Other types include vascular dementia, Lewy body dementia, and frontotemporal dementia. Dementia affects millions worldwide, particularly older adults, though it is not a normal part of aging.

Summary

General Practitioners (GPs) have a low sensitivity (11-12%) in recognizing mild cognitive impairment (MCI) in primary care. Improved tools and training are needed for better detection of this dementia risk factor.

Background:

Purpose of the Study:

  • To evaluate the accuracy of General Practitioners (GPs) in identifying patients with mild cognitive impairment (MCI) within their practice population.
  • To assess the sensitivity and specificity of GPs' clinical judgment in MCI recognition.

Main Methods:

  • A cross-sectional study was conducted using a primary care chart registry sample.
  • 3,242 non-demented patients aged 75-89 years were included.
  • GPs used the Global Deterioration Scale (GDS); MCI cases were defined by a gold standard using psychometric data from trained interviewers.

Main Results:

  • GPs demonstrated very low sensitivity (11-12%) but high specificity (93-94%) in detecting MCI.
  • False negative assignments were more common in patients with higher education levels.
  • False positive assignments increased with patient comorbidity; detection improved when memory or multiple domains were affected.

Conclusions:

  • GPs recognize MCI in a limited number of cases based solely on clinical impression.
  • There is a need for improved, validated, and standardized tools for MCI detection in primary care.
  • Focus should shift towards case-finding approaches and broader cognitive assessments beyond memory.
