Summary

Total shoulder arthroplasty offers better shoulder function than hemiarthroplasty for osteoarthritis patients. However, no other clinical benefits were found, and more research is needed on surgical techniques and comparisons to non-surgical options.

  • Patient-reported outcomes such as pain, function, quality of life, and revision rates were analyzed.
  • Main Results:

    • Seven studies with 238 patients were analyzed. No studies compared surgery to sham or placebo.
    • Total shoulder arthroplasty (TSA) demonstrated statistically significant better functional scores compared to hemiarthroplasty at 24-34 months.
    • No significant differences were found between TSA and hemiarthroplasty for pain, quality of life, or adverse events. A trend towards higher revision rates with hemiarthroplasty was observed.

    Conclusions:

    • Total shoulder arthroplasty appears to provide superior shoulder function compared to hemiarthroplasty, with no other significant clinical advantages identified.
    • Further research is required to compare various surgical components and techniques for shoulder osteoarthritis.
    • There is a critical need for studies comparing shoulder surgery against sham, placebo, and non-surgical treatments.
