Background:

  • Autoimmune hepatitis (AIH) is a chronic liver inflammation driven by immune responses against self-antigens.
  • Genetic susceptibility, particularly HLA-DRB1 alleles, is established for AIH.
  • Specific HLA associations vary geographically, with DRB1*1301 being primary in Brazil.

Purpose of the Study:

  • To investigate additional genetic susceptibility factors for AIH-1 in the extended MHC region in Brazilian children.
  • To analyze the association of specific polymorphisms and haplotypes with AIH-1.

Main Methods:

  • Genotyping of 107 AIH-1 children and 326 healthy controls for polymorphisms in TNFA, LTA, NFKBIL1, BAT1, MICA, and HLA-B.
  • Analysis of linkage disequilibrium and extended haplotypes within the MHC region.
  • Comparison of allele and haplotype frequencies between AIH-1 patients and controls.

Main Results:

  • The TNFA-308 A allele was significantly increased in AIH-1 patients compared to controls.
  • A specific ancestral haplotype (TNFA-308A, TNFA-238G, LTA+252G, LTA+80C, NFKBIL1-63A, BAT1-348C, BAT1-22C, HLA-B*08, MICA*08) was more frequent in DRB1*03 positive patients, conferring a seven-fold increased risk for AIH-1.
  • Diverse class III haplotypes were observed in HLA-DRB1*13 positive patients, indicating genetic heterogeneity.

Conclusions:

  • Extended haplotype analysis highlights genetic diversity within the Brazilian population regarding AIH-1 susceptibility.
  • The findings support the complexity of AIH pathogenesis, suggesting different triggers and HLA presentations contribute to the disease.
  • This study underscores the importance of MHC region microdiversity in autoimmune hepatitis.
