Summary

Tophaceous gout rarely affects the cervical spine. This study reports a rare case of atlanto-axial gout, the fourth documented, and the second managed surgically.

Background:

Tophaceous gout, a manifestation of chronic hyperuricemia, typically affects peripheral joints.
Cervical spine involvement by tophaceous gout is exceptionally rare, with limited case reports in medical literature.

Observation:

This report details a unique case of tophaceous gout specifically involving the atlanto-axial region of the cervical spine.
This represents the fourth documented instance of gouty involvement solely in the atlanto-axial area.

Findings:

The presented case is the second instance of atlanto-axial gout to be managed operatively.
Surgical intervention was deemed necessary for this specific patient.

Implications:

This case expands the understanding of rare spinal manifestations of gout.

It highlights the importance of considering gout in the differential diagnosis of atlanto-axial pathologies, even when uncommon.

Further research may elucidate optimal management strategies for this rare condition.
