Summary

Phacoemulsification offers good visual results in high myopia patients. Myopic retinal degeneration is the main factor limiting vision, not axial length or age. Retinal detachment is rare post-surgery.

Purpose of the Study:

  • Analyze visual results of phacoemulsification in highly myopic eyes.
  • Identify risk factors for poor postoperative visual acuity.
  • Assess the incidence of postoperative complications.

Main Methods:

  • Retrospective review of 209 eyes from 150 high myopia patients.
  • Phacoemulsification and foldable IOL implantation performed between 1998-2008.
  • Analysis of best-corrected visual acuity (BCVA), refractive outcomes, and complications.

Main Results:

  • 63.2% of eyes achieved BCVA of 0.5 or better.
  • 70.3% of eyes were within ±1.0 D of target refraction.
  • Pre-existing myopic chorioretinal macular degeneration was the sole predictor of poor BCVA; age and axial length were not correlated. Retinal detachment (RD) occurred in 0.5% of eyes.

Conclusions:

  • Phacoemulsification provides satisfactory visual improvement in most high myopia cases with low complication rates.
  • Myopic retinal degenerative changes are the primary limitation to visual potential.
  • Lower rates of RD may be linked to reduced Nd:YAG capsulotomy and primary posterior capsulorhexis in younger patients.
