Observation:

  • Diabetes insipidus in children can be an early indicator of GCTs.
  • Cerebral masses may be subtle or undetectable by initial MRI scans.
  • Bifocal masses in the suprasellar and pineal regions strongly suggest germinoma.

Findings:

  • The presence of diabetes insipidus warrants thorough investigation for GCTs, even if initial MRI is negative.
  • Delayed or repeat MRI is crucial for detecting small or developing GCTs.
  • Characteristic imaging findings, like bifocal masses, are highly indicative of germinoma.

Implications:

  • Timely diagnosis of pediatric GCTs is essential for effective treatment and improved outcomes.
  • Recognizing the potential for subtle initial findings in MRI is critical for pediatric neuro-oncologists and endocrinologists.
  • Implementing follow-up imaging protocols can enhance the detection rate of early-stage GCTs.
