Hallucinations and sleep disorders in Parkinson disease (PD) progress differently over 10 years. While hallucinations worsen and often coexist with sleep issues, prior sleep problems do not predict future hallucinations in PD patients.
Area of Science:
- Neurology
- Sleep Medicine
- Neuroscience
Background:
- Parkinson disease (PD) is a neurodegenerative disorder affecting motor function.
- Sleep disturbances and hallucinations are common non-motor symptoms in PD.
- The longitudinal relationship between these symptoms is not well understood.
Purpose of the Study:
- To prospectively evaluate the 10-year progression of hallucinations and sleep disorders in Parkinson disease.
- To determine the relationship between the onset and severity of hallucinations and various sleep abnormalities.
Main Methods:
- A 10-year longitudinal study involving 89 patients with Parkinson disease.
- Assessment of sleep disorders and hallucinations using standardized scales at multiple time points.
- Analysis of longitudinal data using generalized estimating equations.
Main Results:
- Hallucination prevalence and severity significantly increased over the 10-year study period (p < 0.0001).
- Acting out dreams also increased significantly over time (p = 0.001).
- Severe sleep fragmentation was associated with concurrent hallucinations (OR 2.01, p = 0.006), and hallucinations were associated with vivid dreams/nightmares (OR = 2.60, p < 0.0001) and acting out dreams (OR = 2.38, p = 0.0004).
Conclusions:
- Hallucinations and sleep abnormalities exhibit distinct progression patterns in Parkinson disease over a decade.
- While hallucinations in PD patients often coincide with sleep disturbances, pre-existing sleep issues do not predict the future development of hallucinations.
