Ostomy Care01:24

Ostomy Care

Introduction
An ostomy is a surgical procedure that creates an artificial opening from the intestines to the outside of the body, allowing for the rerouting of effluent. This opening is known as a stoma. A stoma usually protrudes above the skin surface, appearing pink or red, moist, and round, and it lacks nerve sensations.
There are different types of ostomies, including colostomies, ileostomies, and urostomies:

Inflammatory Bowel Disease V: Surgical Management

Surgical interventions for inflammatory bowel disease (IBD), which includes ulcerative colitis and Crohn's disease, are essential in managing symptoms and addressing complications. The selection of surgical procedures is contingent upon the specific conditions and complications that stem from these illnesses.
Here are some common surgical interventions for IBD:

Tracheostomy Care I: Pre-procedural Steps

A tracheostomy is a surgical technique that involves making an incision in the neck to provide access to the trachea. It is frequently used in medical conditions such as airway obstruction and prolonged mechanical ventilation. Effective nursing management is crucial for the long-term success of a tracheostomy.

Appendicitis-II: Diagnostic Studies and Management

Diagnosing and managing appendicitis requires a structured and comprehensive approach that spans from initial assessment to postoperative care. Here is an overview of the process:
Diagnosing Appendicitis
It requires a multifaceted approach, starting with a detailed physical examination to pinpoint the location and nature of the pain and identify any associated symptoms. Laboratory tests play a crucial role. A complete Blood Count (CBC) typically reveals leukocytosis (an increased number of...

Assessment of the Gastrointestinal System II: Health Perception Pattern

Assessing the gastrointestinal (GI) system is a complex process that begins with collecting subjective data. This data, collected through patient interviews, provides crucial insights into the patient's health history, perception patterns, and lifestyle habits, all contributing significantly to GI health.
Health Perception Patterns
Health perception patterns offer valuable insights into a patient's lifestyle habits and how they may impact their GI health. These patterns include:

Summary

The Ostomy Skin Tool uses the DET score (discolouration, erosion, tissue overgrowth) to assess peristomal skin issues. A new severity scale aids ostomy nurses in making evidence-based treatment decisions for skin disorders.

Area of Science:

  • Wound care
  • Ostomy management
  • Dermatology

Background:

  • Peristomal skin complications are common in ostomy patients.
  • Standardized tools are needed for objective assessment of peristomal skin changes.
  • The Ostomy Skin Tool (DET score) was previously validated for reliability and validity.

Purpose of the Study:

  • To highlight key findings of the Ostomy Skin Tool validation study.
  • To demonstrate the clinical utility of the DET score through case examples.
  • To introduce a simplified categorical severity scale for improved DET score interpretation.

Main Methods:

  • Utilized the previously validated Ostomy Skin Tool (DET score).
  • Evaluated three clinical cases of peristomal skin changes.
  • Introduced a new categorical severity scale (mild, moderate, severe).

Main Results:

  • The DET score effectively quantifies peristomal skin changes.
  • Case examples illustrate the practical application of the DET score.
  • The simplified severity scale enhances clinical interpretation of the DET score.

Conclusions:

  • The DET score provides an evidence-based method for assessing peristomal skin disorders.
  • The simplified scale aids ostomy nurses in clinical decision-making.
  • Empowers ostomy care nurses with objective data for evaluation and treatment planning.
