    Background:

    • Melanoacanthoma is an uncommon benign skin lesion.
    • Distinguishing it from malignant melanoma is crucial for patient management.

    Observation:

    • Electron microscopy revealed a non-nevoid elevated epithelial tumor.
    • The tumor comprised basaloid and spinous keratinocytes and large dendritic melanocytes.
    • Langerhans cells in the malpighian layers showed normal morphology.

    Findings:

    • Pigment transfer from melanocytes to keratinocytes was incomplete, not completely blocked.
    • Immunofluorescent studies and immunoprecipitin assays excluded a relationship to malignant melanoma.

    Implications:

    • Electron microscopy and immunofluorescence aid in diagnosing melanoacanthoma.
    • These findings help differentiate benign melanoacanthoma from potentially malignant melanocytic lesions.
