Area of Science:

  • Hepatology
  • Gastroenterology
  • Internal Medicine

Background:

  • Chronic hepatitis can present with diverse histological findings.
  • Fatty changes in the liver are often associated with metabolic syndrome or alcohol use.
  • Distinguishing between different causes of chronic liver disease is crucial for patient management.

Purpose of the Study:

  • To characterize a cohort of patients with chronic hepatitis and significant fatty liver changes.
  • To investigate the clinical and histological features of these cases.
  • To determine the long-term prognosis of this specific patient group.

Main Methods:

  • Retrospective analysis of liver biopsy specimens from ten patients.
  • Clinical data collection including patient history, physical examination, and laboratory tests.
  • Histopathological examination of liver biopsies to assess inflammation, fibrosis, and steatosis.
  • Follow-up liver biopsies and clinical assessment for a subset of patients over 1-3 years.

Main Results:

  • Ten patients presented with chronic hepatitis and moderate to severe fatty change on liver biopsy.
  • None of the patients had risk factors like excessive alcohol intake, obesity, diabetes, or steroid use.
  • Mallory's hyalin was absent, differentiating these cases from typical alcoholic hepatitis or non-alcoholic steatohepatitis.
  • Follow-up in eight patients revealed a benign clinical course with no progression to cirrhosis.

Conclusions:

  • This study describes a unique group of patients with chronic hepatitis and fatty liver, distinct from common etiologies.
  • The findings suggest a potentially favorable prognosis for this specific presentation of chronic liver disease.
  • Further research, including serological testing for non-A, non-B hepatitis, is needed to elucidate the etiology.
