Summary

Elderly patients with heart failure experience significantly more intense thirst than others. This intense thirst should be recognized as a key symptom of worsening heart failure in older adults.

Background:

  • Elderly individuals often have a diminished sense of thirst.
  • Thirst is a common yet poorly understood symptom in elderly patients with heart failure (HF).

Purpose of the Study:

  • To compare thirst intensity in elderly patients with and without heart failure (HF).
  • To assess the relationship between thirst, health-related quality of life (HRQoL), and fluid balance in HF patients.

Main Methods:

  • Compared thirst intensity and HRQoL in 48 elderly patients (mean age 80) with worsening HF (n=23) versus other acute illnesses (n=25).
  • Measured serum sodium and analyzed urine for color, sodium concentration, and osmolality.

Main Results:

  • HF patients reported significantly higher thirst intensity (median 75 mm) than controls (median 25 mm; p < 0.0001).
  • No significant correlation was found between thirst and HRQoL.
  • HF patients' urine showed lower sodium concentration and osmolality compared to controls.

Conclusions:

  • Elderly patients with worsening heart failure experience considerably increased thirst.
  • Intense thirst should be considered a significant symptom of heart failure in the elderly population.
