Summary

Argon plasma coagulation (APC) tonsillectomy in children significantly reduces operative time and blood loss. This safe, effective technique offers comparable postoperative pain and hemorrhage rates to conventional methods.

Area of Science:

  • Otolaryngology
  • Minimally Invasive Surgery
  • Pediatric Surgery

Background:

  • Argon plasma coagulation (APC) utilizes ionized argon gas for effective hemostasis and controlled tissue coagulation.
  • APC presents a potentially advantageous alternative to traditional tonsillectomy due to its precise application and limited tissue penetration.
  • Pediatric tonsillectomy remains a common procedure with established methods and associated morbidities.

Purpose of the Study:

  • To prospectively compare "hot" APC tonsillectomy with conventional "cold" dissection tonsillectomy in pediatric patients.
  • To evaluate differences in operative time, intraoperative bleeding, and postoperative morbidity.
  • To assess the safety and efficacy of APC as a tonsillectomy technique in children.

Main Methods:

  • A prospective, randomized trial involving 218 pediatric patients (aged 4-15 years).
  • Patients were randomized into two groups: APC tonsillectomy (n=109) and conventional cold dissection tonsillectomy (n=109).
  • Outcome measures included operative time, intraoperative blood loss, postoperative pain (visual analogue scale), and hemorrhage incidence.
  • Main Results:

    • APC tonsillectomy demonstrated significantly reduced operative time and intraoperative blood loss (P < .001).
    • No statistically significant differences were observed between the APC and cold dissection groups regarding postoperative pain intensity.
    • The incidence of postoperative primary and secondary hemorrhage was comparable between both surgical techniques (P > .05).

    Conclusions:

    • Argon plasma coagulation tonsillectomy is a safe, easy, and effective technique for pediatric patients.
    • APC offers reduced operative time and minimal intraoperative blood loss compared to conventional cold dissection.
    • The cost-effectiveness and comparable postoperative outcomes make APC a viable alternative for tonsillectomy.
