Summary

A rare case of pneumothorax in an 82-year-old man was linked to lymphoplasmacytic lymphoma. Lymphoma involvement of the pleura, undetected by imaging, caused this complication, successfully treated with surgery.

Background:

  • Lymphoplasmacytic lymphoma (LPL) is a rare B-cell neoplasm typically affecting lymph nodes and bone marrow.
  • Initial radiological examinations in this patient revealed no pulmonary or pleural involvement.
  • The patient received rituximab for diagnosed LPL.

Observation:

  • Eighteen months post-treatment, the patient developed a right pneumothorax.
  • Surgical intervention, including bullectomy, was performed.
  • Histological analysis revealed abnormal B lymphocyte proliferation near the bulla and pleura.

Findings:

  • The pneumothorax was directly associated with lymphoplasmacytic lymphoma.
  • Pleural involvement by lymphoma, though radiologically occult, was identified as the cause.
  • Abnormal B lymphocytes were found infiltrating the pleura and bulla wall.

Implications:

  • This case highlights that lymphoma can involve the pleura even when not detected by standard radiological imaging.
  • Pleural lymphoma is a potential cause of spontaneous pneumothorax.
  • Surgical management is an effective treatment for lymphoma-associated pneumothorax.
