Summary

A rare prostate multilocular cystadenoma case is presented in a 36-year-old man with primary infertility and obstructive azoospermia. Surgical removal confirmed the diagnosis, highlighting the need for further research into prostatic cystic lesions.

Background:

Prostate multilocular cystadenoma is a rare benign cystic lesion.
Cystic lesions of the prostate can present with varied symptoms, including infertility.
Obstructive azoospermia can be a consequence of prostatic abnormalities.

Observation:

A 36-year-old male presented with primary infertility and mild pelvic pain.
Semen analysis showed azoospermia with positive fructose.
Imaging revealed a large multilocular cystic mass in the prostate.

Findings:

Transrectal ultrasonography-guided biopsy showed regular low cuboidal cells.
Surgical resection via a transperitoneal/retroperitoneal approach was performed.
Pathology confirmed the diagnosis of prostatic multilocular cystadenoma.

Implications:

This case expands the understanding of prostatic multilocular cystadenoma presentations.
Highlights the association between prostatic cystic lesions and male infertility.
Further studies are warranted to classify and characterize prostatic cystic lesions.
