Early steroid withdrawal (ESW) in kidney re-transplantation (KRT) is safe and effective. This approach offers comparable patient and graft survival to chronic steroid maintenance (CSM), with fewer side effects.
Area of Science:
- Nephrology
- Transplantation Immunology
- Immunosuppression Therapy
Background:
- Kidney re-transplantation (KRT) candidates face high graft failure risks.
- Chronic steroid maintenance (CSM) is standard for KRT patients.
- Safety of early steroid withdrawal (ESW) in KRT is not well-established.
Purpose of the Study:
- To compare outcomes of ESW versus CSM in KRT recipients.
- To evaluate patient survival, graft survival, and graft function.
- To assess the incidence of adverse events associated with each regimen.
Main Methods:
- Retrospective analysis of 113 KRT patients.
- Groups: ESW (n=59) and CSM (n=54).
- All patients received induction therapy and were maintained on mycophenolate/tacrolimus.
Main Results:
- No significant differences in 1- and 5-year patient or graft survival between ESW and CSM groups.
- Comparable estimated GFR at 1 and 5 years between groups.
- CSM group had higher rates of hyperlipidemia, osteoporosis, post-transplant diabetes, and hypertension requiring more medications.
Conclusions:
- ESW is a viable immunosuppression strategy for KRT recipients.
- ESW demonstrates comparable short- and intermediate-term outcomes to CSM.
- ESW may reduce the incidence of steroid-related comorbidities.
