Background:

  • Spontaneous arterial dissection is a rare condition.

Observation:

  • A 58-year-old female presented with acute left lower abdominal pain.
  • CT angiography revealed proximal IMA dilation, distal occlusion, and peri-arterial edema.
  • Laparotomy confirmed IMA dissection with secondary thrombosis.

Findings:

  • This is the first reported case of spontaneous isolated IMA dissection.
  • Surgical intervention included thrombectomy, intimal flap resection, and autogenous vein patch reconstruction.
  • Successful arterial reconstruction was achieved.

Implications:

  • Isolated IMA dissection should be considered in the differential diagnosis of acute abdomen.
  • Radiological findings like arterial dilation, occlusion, and peri-arterial edema are key indicators.
  • High-resolution CT can aid in diagnosing IMA dissection by visualizing intimal flaps and double lumens.
